Dr Cullen is not registered as a health practitioner of any type. His qualifications (with the exception of the MFM, Master of Family Medicine, from Monash university) are from the University of Auckland.

First, the blood tests
It is very useful to have some blood tests done before breakfast a few days before your initial consultation. The tests required are fasting glucose, fasting insulin, and HbA1C (haemoglobin A1C). You will have to pay for these tests at the time the blood is taken. The cost will be about $110.
In Auckland, community laboratory services are provided by LabTests, www.labtests.co.nz. Their website has a list of blood collection centres, and each collection centre has forms for self-requested blood tests. A form can be printed here These are fasting blood tests so please ensure that nothing other than water passes your lips for ten hours before the test. No smoking during the fasting period. The laboratory will email the test results to you. Please forward the results to email@example.com and ask the laboratory to copy the results to firstname.lastname@example.org
It is also useful if you have copies of your previous blood tests sent to, or with you to show to, Dr Cullen.
Labtests is required by section 22F of the Health Act to send copies of your test results to whomsoever you choose. Section 22F(1) reads "Every person who holds health information of any kind shall, at the request of the individual about whom the information is held, or a representative of that individual, or any other person that is providing, or is to provide, services to that individual, disclose that information to that individual or, as the case requires, to that representative or to that other person "
The initial consultation
You will be asked to complete a medical history questionnaire and to sign a form acknowledging that you understand Dr Cullen is not registered as a health practitioner of any kind.
You can expect Dr Cullen to begin the consultation by asking a number of questions about your history as this relates to diabetes or pre-diabetes, and about your daily eating and exercise routines, if you have any.
Then there will be a discussion with you about type 2 diabetes or pre-diabetes, its causes, and the general approaches to reversal.
Finally, having heard what options you prefer, Dr Cullen will suggest a plan that aims to reverse your diabetes or pre-diabetes. It is unlikely that Dr Cullen will ask to examine any part of you.
You can expect to receive a summary letter in the post or by email within a day or so of the consultation. Dr Cullen anticipates that many people will want to discuss this with their family, GP, or nurse.
We collect payment ($100) prior to consultations. We accept payment by cash or credit card. We do not have an eftpos facility. If you are unable to pay, or wish to pay by instalments, please let us know when you make your appointment.
Credit card payments are accepted through Paypal.
Email and Skype
Not everybody lives in Auckland. Dr Cullen is prepared to offer advice by email, or to consult via Skype. There is no fixed fee for either of these. We do welcome any donation to our charity, Tamaki Education and Sports Society Inc. Donations are tax deductible (in New Zealand) and can be made through the Paypal button below.
